Conduct Policies for a Positive Campus

Our office has several different kinds of forms that can be conveniently accessed online and downloaded.Be

sure to read all of the accompanying instructions and that the forms have been filled out completely. Please select from the following forms below:

• Obtain copies of documents from your student conduct file.
• Allow others to access or discuss or your student disciplinary case/history.
• Report a hazing incident.
• Refer a student for academic misconduct.
• Refer a student for a non-academic conduct issue.
• Confirm your organization's attendance at an assigned workshop.
• Refer a student living in the adjacent area surrounding UCR for failing to follow the Good Neighbor Guidelines.
• Request an Educational Presentation for Students, Faculty or Staff.
• Apply to become a member of the Student Conduct or College Academic Integrity Committee.
