Academic Misconduct Report Form 


 

The Academic Misconduct Referral Form

Succesful completion of the Academic Misconduct Referral Form should include all of the following:

  • Be certain to include the student ID number, as there are multiple students with the same name on campus.

  • Refer to the Academic Integrity definitions approved by the Academic Senate in order to designate the type of alleged violation.

  • When a student admits responsibility, they should sign the form in agreement. A student who admits responsibility is admitting that they have violated the Academic Integrity policy.

  • When a student has admitted responsibility in writing (for example, via email), please attach the email or other correspondence with the student. It is preferred that the student's email state that they admit they have committed a policy violation.

  • When a student has failed to respond to attempts to communicate them, where possible, please include documentation of the attempt(s) in the form of copies of the letters/emails, and/or a call log documenting dates/times of attempted telephone contact. Include the telephone number contacted.

  • When the instructor and student meet, the form should be properly signed by the student and instructor regardless of an admission of responsibility or a denial of responsibility in order to document the outcome of the meeting.

  • To ensure timely processing, please ensure the form is completed and documentation of the alleged violation(s) is attached or enclosed when sending the refferal to SCAIP.
